What is it?

MicroTESE, or microsurgical testicular sperm extraction, is an advanced microsurgical technique for obtaining sperm directly from the testes (testicles). It is the most successful treatment for men with non-obstructive azoospermia (NOA). By using a high-powered surgical microscope, our specialists can search for tiny areas in the testes that might still be producing sperm, even when there are no sperm in the ejaculate.

Why is it done?

Approximately 1 in 100 men have azoospermia, which is when there are no sperm in the ejaculate (the semen). For some men this is due to a blockage in the transport of sperm (obstructive azoospermia), whilst for others it is a problem with sperm production itself (non-obstructive azoospermia).

MicroTESE can be offered to men with non-obstructive azoospermia who wish to conceive, and offers them the highest chance of successful sperm retrieval.

What is involved?

During an initial consultation with one of our specialists in male infertility, we will undertake a comprehensive assessment of factors that could be contributing to infertility or causing azoospermia. Usually, two semen analyses will be performed to confirm that no sperm are present. Various blood tests and other investigations are ordered to look for underlying causes. Once the diagnosis of non-obstructive azoospermia is confirmed, our specialists will discuss treatment options with you, which may include hormonal treatments and/or surgical management.

The microTESE procedure is performed under a general anaesthetic in our onsite day hospital. A small cut is made in the scrotum to allow access to the testis. Your surgeon will carefully examine the testicular tissue under a high powered microscope to identify any areas that may still be producing sperm; these areas are collected and immediately examined by a laboratory scientist to confirm if sperm are present. Any sperm that are found can either be used to fertilise eggs immediately or frozen for later use.

You will generally be able to go home the same day.

What are the risks?

MicroTESE is generally safe, however as with all surgical procedures there are risks involved. It is normal to experience some discomfort, swelling or bruising after the procedure, and most people make a full recovery over 1-2 weeks. Serious complications are rare and include infection or bleeding within the scrotum (a scrotal haematoma), which could require antibiotics or further surgical management. Your specialist will discuss these with you in detail prior to the procedure.
